Project Coordinator and Project Assistant - Care and Support in the Home (Reading)

We have two exciting new opportunities as part of our Care and Support project.

Project Coordinator:
Hours: 20 per week over 4/5 days to be negotiated
Salary: £9,736 – 2 year fixed term contract

Project Assistant:
Hours: 12 per week over 3 days to be negotiated
Salary: £3,683 – 2 year fixed term contract
Reference: SE48956

This new and rewarding project involves providing support and assistance to older people at home after discharge from hospital, helping them to restore and maintain their independence. We require an enthusiastic and motivated team to ensure the success of the project. The team will be working with local agencies, such as the hospital, social services and other voluntary agencies to identify patients who would benefit from low-level support in their own homes for up to a two-week period.

Experience of working with older and vulnerable groups would be an advantage as would project development experience for the Project Coordinator role. The successful candidates will be able to travel as appropriate to the needs of the job and will have access to a car. They need to be proactive in raising awareness of the service, attending meetings and giving presentations and talks as required. They will also be able to respond to emergency situations and be part of emergency call out procedures.
